New Patients - First Appointment
Prior to First Appointment we will need the "New Patient" Packet completed - this includes demographics, health history, etc.
You will not be able to see the Provider without All New Patient Paperwork completed.

All Forms must be completely filled out prior to Appointment.
Filling the forms out in advance allows you to carefully consider the questions, provide complete answers, and it saves valuable time during the appointment. You may also bring any supplemental information that you feel is important.
Established Patients
Please Arrive 10-15 minutes prior to your Scheduled Appointment Time - this allows for check-in, information updating so that you can be in the room or video link ready to be seen by Provider at your scheduled appointment time.
Please let us know if there are any changes to your personal, insurance, or health information.
Print the appropriate forms below and fill in the updated information. Bring the form and any other related information to your next visit or email/fax to clinic so we can update our records.
This will allow us to provide the best care possible, and reduce errors in billing.
Prescription Refills
To request a refill, contact your pharmacy. The pharmacist will contact the Clinic for you and can obtain authorization for prescription refills from 9:00 a.m. to 4:00 p.m., Monday through Friday. Make sure you Request Refills at Least 4 Days Prior to running out - This allows time in case your Provider is not in the office that day or for the weekend. The clinic does not provide prescription refills after hours. If you need a change to your dosage or quantity of medication, please contact the clinic and leave a detailed message with the Medical Assistant for your provider.
After Hours Care
Patients needing urgent medical care when the Clinic is closed may reach the on-call provider by dialing 503-689-8460 to reach our Answering Service, who will contact the provider on call. The On-Call Provider will offer Advice Only regarding your Symptoms. The On-Call Provider Does Not Prescribe or Refill Medications. The On-Call Provider Does Not have access to your Chart or any Messages left for you by our Staff.
When calling after hours, remove any incoming call blocks from your phone line so that phone calls can be received. All after-hour calls are logged by the answering service and are accessible upon request. These records are kept according to State and Federal regulations.
Co-Payments/Deductibles
All insurance co-pays/deductibles are due the day of service. Please come prepared. This is a requirement of your insurance company.
No Show Policy
A $50 No Show fee will be charged to those who fail to appear on time for a scheduled appointment.
The No Show fee will also apply to cancellations made less than 24 hours before the scheduled appointment.
Patient Forms -

Photo ID, Insurance Card and All Current Medication Bottles.
